Registration Information

Parents, please get all your completed forms to your manager or site coordinator.  It is urgent for us to complete the registration process.

The following forms must be completed this week.

  • Copy of Birth Certificate (if you played with Fusion FC last season, then you team manager has this document)

  • TSSA Medical Release (must be notarized and the notary commission must be valid until 6/15/10) *** For all teams U14 and up. ***

  • US Club Soccer Medical Release (must be notarized and the notary commission must be valid until 6/15/10) *** For all teams U12 and down and U13 Select. ***

  • Photo Release

  • Small Photograph (for player pass)

  • Player Agreement

  • Bristol Release - This is a waiver from the city of Bristol, TN that must be signed by all players.

  • Scholarship Form - Scholarship money is available and is based upon need. If you feel that you have a need, please complete form and mail to address in page footer by September 1st, 2009 or email to chad.craft@fusionsoccer.com.
