Forms

Forms are provided in PDF(portable document format) format. Some PDF Forms can be filled out on-line and then printed for your use or saved to disk.  For best results when viewing the PDF version you must have Adobe Acrobat Reader 4.0 or higher. To save the information you type in, you must have Adobe Acrobat version 4.0 or higher.

• TSSA Medical Release (must be notarized and the notary commission must be valid until 6/15/10) *** For all teams U14 and up. ***

• US Club Soccer Medical Release (must be notarized and the notary commission must be valid until 6/15/10) *** For all teams U12 and down and U13 Select. ***

• Photo Release

• Small Photograph (for player pass)

• Player Agreement

• Bristol Release - This is a waiver from the city of Bristol, TN that must be signed by all players.

• Scholarship Form - Scholarship money is available and is based upon need. If you feel that you have a need, please complete form and mail to address in page footer by September 1st, 2009 or email to chad.craft@fusionsoccer.com.
